Each of 10 severity items: 0-4. Obsessions: 0-20. Compulsions: 0-20. Total = sum of all 10 items (0-40). No score if ratings are incomplete.

Commonly used severity interpretation: 0-7 subclinical, 8-15 mild, 16-23 moderate, 24-31 severe, 32-40 extreme. Severity bands vary slightly by reference and population.
